Prime Minister Trudeau, as health professionals in Canada, we are asking you to acknowledge that lives lost to climate change and air pollution are every bit as valuable as those lost to COVID-19 by making life-saving commitments as the Leaders’ Summit on Climate on Earth Day 2021 approaches.

Last spring, organizations representing two-thirds of health professionals worldwide signed an open letter to G20 leaders asking you to tackle the health crises of COVID-19 and climate change simultaneously in your economic recovery spending. This was the largest known mobilization of global healthcare professionals in history.

Climate change is worsening asthma and evacuations from wildfires, increasing deaths from heat waves, making allergy seasons longer and more severe, posing challenges to food security, hastening the spread of Lyme Disease, and raising the potential for new pandemics. Related disruptions to infrastructure, supply chains, and staffing pose risks to the stability of healthcare systems themselves.

The same measures that can help Canada achieve net zero by 2050 can save an estimated 112,000 lives from air pollution alone. Greenhouse gases can be decreased with changes that also reduce air pollution; increase physical activity; improve our diets; increase our access to nature; and reduce the local toxic impacts of resource extraction, which disproportionately impact low-income, Indigenous, and racialized peoples.

Nonetheless, since last year’s Healthy Recovery mobilization:

• Environment and Climate Change Canada reported Canada’s greenhouse gas emissions increased in 2018. These were largely unchanged between 2000-2018, while the United Kingdom’s dropped by 35%, and the United States by 8%.
• Health Canada reported 15,300 people in Canada died of air pollution in 2016, and a Harvard study concluded that 20% of global premature deaths are due to fossil fuel-related air pollution.
• Canada doubled-down on subsidizing fossil fuel energy, with $20.74 billion dollars, or $551.87 per capita allocated since the beginning of the pandemic.

The world is currently headed for over 3 degrees Celsius of warming within the lifetime of today’s children, which poses potentially catastrophic risks to health and health systems. Therefore, we urge you to:

1. Go big in Canada’s next commitment to the Paris Agreement.
A healthy Nationally Determined Contribution to the Paris Agreement is one which protects the health of today’s children by representing Canada’s fair share of global commitments to keep the world well below 2 degrees Celsius of warming, and, as much as possible, is achieved via measures that strengthen health, wellbeing, and healthcare systems immediately.

2. Hold us accountable by strengthening Bill C-12.

• The first reporting year for Bill C-12 must be 2025.
• An independent body of scientific experts with their own secretariat and climate modelling ability, who report to Parliament, not to the Minister of the Environment, is necessary in order to audit alignment of policy with climate goals without fear of reprisal.
• Bill C-12 must be explicitly aligned with the United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP).

3. Eliminate fossil fuel subsidies.
Imagine if we had spent an additional $551.81 per capita over the past year helping fossil fuel workers transition to a caring and low-carbon economy instead of increasing deadly fossil fuel-related pollution? Given what we now know about the health risks of air pollution, subsidizing fossil fuels is as unacceptable as subsidizing tobacco.

4. Pass Bill C-230, an Act respecting the development of a national strategy to redress environmental racism
The local toxic impacts of resource extraction on Indigenous and racialized people in Canada have been understudied and under-addressed. This is unconscionable. A new approach is required.

As we saw with the onset of COVID-19, when people in Canada understand urgent risks to our health, we support rapid change and strong leadership in order to minimize them. Like us, you carry the responsibility of protecting the health of our children, grandchildren, and future generations. Let Earth Day 2021 be the day we apply hard-won lessons from our current planetary health emergency to help us save lives from the next.
